Career after MBBS in Russia: India, NEET-PG, the USA, the UK and the Gulf

Every rule on this page was read from its source document on 18 October 2026.

Short answer

A Russian MBBS leads to practice in India only if the degree meets the National Medical Commission’s (NMC) Foreign Medical Graduate Licentiate Regulations, 2021 (FMGL-2021). After that come the Foreign Medical Graduate Examination (FMGE), a 12-month internship in India and permanent registration with a State Medical Council. The National Exit Test (NExT), meant to replace FMGE, has stood deferred since 13 July 2023. You can sit NEET-PG, the entrance test for MD (Doctor of Medicine) and MS (Master of Surgery) seats in India, once you have passed FMGE and hold a registration, as long as your internship ends by that year’s cut-off.

Abroad, the university counts for more than the country. For the USA, its entry in the World Directory of Medical Schools (WDOMS) needs a Sponsor Note from the Educational Commission for Foreign Medical Graduates (ECFMG) covering your graduation year. For the UK, the General Medical Council (GMC) has to accept your degree before you sit the Professional and Linguistic Assessments Board (PLAB) test, and it assesses seven Russian institutions case by case. For India, one weak point is the internship: Russia’s standard for General Medicine builds practice into six years and names no separate internship year, so get the split in writing before you join. FMGL-2021 also wants the licence to practise in Russia. That comes through primary accreditation, an assessment held in Russian, and we found no NMC document that says it meets NMC’s condition, so ask the university how its foreign graduates obtain it.

Exam dates, cut-offs and listings change. Read the current bulletin or listing before you act on any of them.

What the Russian degree is, on paper

Russia’s federal standard for General Medicine is the Federal State Educational Standard (FGOS) for 31.05.01 General Medicine, approved by the Ministry of Science and Higher Education’s order No. 988 of 12 August 2020 and amended on 26 November 2020 (we have not checked a later amendment of 27 February 2023). It sets the programme at six years, counting the vacation after the State Final Attestation, and 360 credit units (clauses 1.9 and 1.10).

Clinical practice sits inside those six years. Block 2, “Practice”, is worth at least 45 of the 360 credits, and clause 2.4 lists the types of practice, among them therapeutic, surgical, obstetric-gynaecological and general-practitioner profile; each university picks which of them it runs (clause 2.6). Block 3, the State Final Attestation, is preparing for and passing the state examination (clause 2.7). Neither the practice nor the State Final Attestation may be done by distance learning (clause 2.8). The standard names no separate internship year.

Why that matters for India

FMGL-2021 regulation 4(a) asks for a course of at least 54 months and an internship of at least 12 months at the same foreign institution, and Schedule I says the internship is “in addition to such course”. NMC’s FMGL FAQs (question 17) put it plainly: clinical posting is part of the 54-month course, while the internship “is done after the completion of MBBS course”. The FAQs warn that they cannot be upheld in a court of law, but Schedule I’s wording points the same way. Six years is longer than 54 plus 12 months, but the federal standard does not divide it that way. The division has to come from the university, in writing, before you join.

Language is the second check. Clause 1.8 of the standard says the programme is taught in Russian, the state language, unless the university’s own internal regulation (a “local normative act”) provides otherwise. FMGL-2021 regulation 4(a)(iii) wants English as the medium of instruction. Ask which university document makes your programme English-medium, and whether any year, clinical rotation or the internship switches to Russian. Our page on English-medium MBBS in Russia goes into this.

Practising in India

Regulation 3 of FMGL-2021, notified on 18 November 2021, says no foreign medical graduate may practise medicine in India unless granted permanent registration under those regulations. Your admission date decides which rules you follow. If you were already studying abroad before 18 November 2021, regulation 5 leaves you under the earlier rules, which NMC’s notice of 18 March 2026 names as the Screening Test Regulations, 2002. If you were admitted on or after that date, the steps run in this order:

  1. Finish the course and the 12-month internship at the same Russian university. No part of the course, training or internship may be done in India or a third country, and the course must be finished within ten years of joining (FMGL-2021 Schedule I, 2(iii) and 2(iv)).
  2. Hold the licence to practise in Russia. Regulation 4(b) asks for registration with the body that licenses doctors in the country that awarded the degree, “at par with the license to practice medicine given to citizen of that country”. The FMGL FAQs repeat it (question 9) and list it among the conditions for starting the internship in India (question 16). In Russia that licence comes through primary accreditation, described under Practising in Russia. We found no NMC document that names Russian accreditation as meeting this condition, so ask the university how its foreign graduates obtain it, and keep the record.
  3. Pass FMGE. The National Board of Examinations in Medical Sciences (NBEMS) decided in its notice of 28 August 2026 to hold it three times a year instead of twice. The October 2026 session is on 31 October 2026, with the result due by 30 November 2026. The same notice moves the session NBEMS had set for 9 January 2027 to a later date in January or February 2027, which it had not notified by 18 October 2026. For admissions on or after 18 November 2021, the October 2026 bulletin (clause 4.14) says NBEMS decides eligibility strictly under FMGL-2021 and declares a non-compliant candidate ineligible.
  4. Do a 12-month internship in India under the Compulsory Rotating Medical Internship (CRMI) Regulations, 2021. You apply to your State Medical Council through NMC (FMGL FAQs, question 15). Before you start, you need provisional registration for the internship from that council, one of the conditions question 16 lists. You must finish within two years of passing FMGE (CRMI Schedule I, 2(a)).
  5. Apply for permanent registration. CRMI regulation 4 makes no medical graduate eligible for it without the internship in India.

An FMGE pass does not register you. The bulletin says sitting the exam gives no automatic right to provisional or permanent registration, and clause 4.15 leaves the decision to the State Medical Council when you apply. The exam itself, its pattern and its pass mark are in our FMGE exam guide, and each Russian university’s FMGE record, with candidate counts, is on FMGE pass rates by university.

Internship seats and stipend

Under Schedule II of the CRMI Regulations, foreign graduates must do the internship at par with Indian graduates, and only in colleges approved for CRMI. They may be posted first in newly opened colleges that are not yet recognised, and may be placed through a merit-based counselling or seat-allocation process. A college may give foreign graduates up to 7.5% of its permitted intern quota.

NMC’s circular of 10 March 2026 has State Medical Councils and the state Directorates of Medical Education allot the slots: 7.5% of permitted intake in established colleges, 100% in new colleges, and 100% of the seats added between 2022-23 and 2025-26. Its Annexure-I counts 43,250 slots for 2026-27, leaving out the Institutes of National Importance: 6,748 at 7.5% of the 2021-22 seats plus 36,502 added seats. The circular counts these on top of seats in listed non-teaching hospitals. NMC’s one-time permission for foreign graduates to intern in those hospitals ran to May 2026, and we found no extension when we searched NMC’s notices on 18 October 2026.

The CRMI Regulations set no stipend amount. Schedule IV says interns are paid the stipend “as fixed by the appropriate authority applicable to the institution/University or State”, for 52 weeks only, so ask the college or the state what it pays. On 24 July 2026 the Government told the Lok Sabha that NMC circulars entitle foreign graduates in CRMI to a stipend set and paid by the state authorities, and that a Supreme Court interim order in writ petition (civil) No. 793 of 2025 says they are entitled to it at par with Indian students. Our guide to the internship in India after MBBS abroad covers the application.

The ten-year limits

NMC sets a 10-year limit in three places, each counted from the day you joined or were admitted to your Russian university, and each worded differently. FMGL-2021 (Schedule I, 2(iv)) wants the whole course finished within ten years of joining. NMC’s public notice of 7 December 2023 adds, in item 5: “All FMGs shall have to complete their internship within 10 years from date of joining their Foreign Medical Institutions”. NMC’s FAQ of 8 June 2026 (question 12) reads that clause with the Graduate Medical Education (Amendment) Regulations, 2019, so a graduate admitted abroad after 2019 must clear the FMG examination within 10 years of admission. Plan as if all three apply.

Where NExT stands

NMC’s NExT Regulations, 2023, notified on 27 June 2023, make NExT both the licensing exam and the entrance exam for broad-specialty postgraduate seats, and foreign graduates who meet FMGL-2021 must sit it. Once NExT is in force the equivalent exams are to be phased out, though NMC may keep them running for earlier batches “for such time as deemed appropriate”.

On 13 July 2023 NMC deferred NExT on the Health Ministry’s advice of 11 July 2023, “till further directions”. When we searched NMC’s What’s New list on 18 October 2026, the latest NExT item was a notice of 8 February 2024 extending comments on the 2023 regulations to 17 February 2024. No start date or first batch has been notified.

This leaves NMC’s 2023 registration rules pointing at an exam that has not started. Regulation 3(ii) of the Registration of Medical Practitioners and Licence to Practice Medicine Regulations, 2023, notified on 10 May 2023, makes a foreign graduate’s provisional registration for the internship depend on passing NExT. Regulation 4 keeps the “prevailing system of Registration” running until those provisions are in force. NMC’s draft amendment of 11 August 2026 would write in FMGE “until National Exit Test (NEXT) comes into force”. It is a draft, open to objections for 30 days, and on 18 October 2026 What’s New showed no final version.

One listing on What’s New misleads: the item dated 16 April 2026 and titled as the registration “(Amendment) Regulations, 2026” is an earlier draft about Armed Forces doctors, and the August draft withdrew it. Plan for FMGE.

MD or MS in India: NEET-PG

NBEMS describes NEET-PG as “an eligibility-cum-ranking examination prescribed as the single entrance examination” for MD, MS and PG Diploma seats, under section 61(2) of the NMC Act, 2019. The All India Institutes of Medical Sciences (AIIMS), the Postgraduate Institute of Medical Education and Research (PGIMER), the Jawaharlal Institute of Postgraduate Medical Education and Research (JIPMER), the National Institute of Mental Health and Neuro Sciences (NIMHANS) and Sree Chitra Tirunal are outside its centralised MD and MS admissions.

The NEET-PG 2026 bulletin shows what a Russia graduate has to hold. Under clause 4.10 (page 30), an Indian citizen or Overseas Citizen of India (OCI) with a medical degree from abroad must have qualified FMGE under the Screening Test Regulations, 2002, must be registered with NMC, the former Medical Council of India or a State Medical Council, and must have completed the internship or be due to complete it on or before 30 September 2026. So you can apply while the internship in India is still running, as long as it ends by the cut-off. The rest of the bulletin is strict about it:

  • ›Clause 4.3: an application from anyone finishing the internship after 30 September 2026 is summarily rejected.
  • ›Clause 4.4: the internship dates you enter are final, and the original internship completion certificate is checked at counselling or admission.
  • ›Clause 4.6: proof of registration is checked on exam day and again at counselling or admission.
  • ›Page 30: appearing in NEET-PG confers no automatic right to admission.
  • ›Page 93: in a tie, your FMGE percentage is used where an Indian graduate's aggregate across the MBBS professional exams would be. It applies only after six earlier tie-breakers that compare how many wrong (negatively marked) answers candidates gave.

The 2026 bulletin scheduled the exam for 30 August 2026, with applications from 1 to 21 July 2026 and results by 30 September 2026. The internship cut-off moves with each session, so time your internship in India against the bulletin for your own year.

The USA: ECFMG Certification and the USMLE

The Accreditation Council for Graduate Medical Education (ACGME) requires certification from the Educational Commission for Foreign Medical Graduates (ECFMG), now a division of Intealth, of every foreign graduate who enters a US residency or fellowship programme it accredits. Certification does not guarantee a place. ECFMG’s 2026 Information Booklet starts with the school: its WDOMS entry must show an ECFMG Sponsor Note on the Sponsor Notes tab, and “if there is no ECFMG Sponsor Note” you are not eligible for ECFMG Certification. Your graduation year must fall within the years the note covers, and you need credit for at least four credit years from a school listed with a Sponsor Note. ECFMG verifies every diploma with the school that issued it and asks the school for the final transcript.

Then the exams. You submit an Application for ECFMG Certification and pass the United States Medical Licensing Examination (USMLE) Step 1 and Step 2 CK (Clinical Knowledge). For clinical and communication skills you complete an ECFMG Pathway, which includes a satisfactory Occupational English Test (OET) Medicine score. Every Pathway applicant takes OET, whatever their first language or medium of instruction. A valid pass on the former Step 2 CS (Clinical Skills) exam also meets this requirement. To take part in the 2027 National Resident Matching Program (NRMP) Match, you must meet the exam requirements by the NRMP Rank Order List certification deadline, and a 2027 Pathways application must be in by 31 January 2027, US Eastern Time.

Step 3 comes after certification. The USMLE program requires ECFMG Certification and a valid, unexpired certificate for it, and recommends completing, or nearly completing, at least one year of US postgraduate training first. Steps 1 and 2 CK are open earlier, to students and graduates of a school that WDOMS lists as meeting ECFMG’s eligibility requirements.

Accreditation, WFME and Russia

Intealth began implementing a Recognized Accreditation Policy in November 2024. A school meets it if it is accredited by an agency recognised by the World Federation for Medical Education (WFME) or by the US National Committee on Foreign Medical Education and Accreditation (NCFMEA), and it also holds an ECFMG Sponsor Note. Intealth says the policy does not affect eligibility for ECFMG Certification or the USMLE, or a school’s eligibility for a Sponsor Note. Where a school meets it, WDOMS shows a notation on its Sponsor Notes tab, and Intealth says a general statement about the policy will be added to ECFMG Status Reports in the Electronic Residency Application Service (ERAS) in the future.

On WFME’s list of recognised agencies, checked 18 October 2026, the only row for the Russian Federation is the Independent Agency for Accreditation and Rating (IAAR), domiciled in Kazakhstan, recognised until January 2028. No Russia-based agency is on it, and Russia is not among the countries with agencies applying. India’s NMC is recognised until September 2033. Neither Russia nor India is on NCFMEA’s list of comparability decisions.

Here is how it looks for one university. Privolzhsky Research Medical University’s WDOMS entry (F0000289) shows an ECFMG Sponsor Note for graduation years 1953 to current, and records IAAR accrediting the programme on 20 June 2025. On 18 October 2026 its Sponsor Notes tab carried Canada and ECFMG notes and no Recognized Accreditation Policy notation. The entry for Petre Shotadze Tbilisi Medical Academy (F0000571), in Georgia, shows what that notation looks like. We have not checked the other Russian universities, and WDOMS has to be read school by school.

WDOMS itself says a listing “does not denote recognition, accreditation, or endorsement” by the directory or its publishers, WFME and the Foundation for Advancement of International Medical Education and Research (FAIMER). What matters for the USA is the Sponsor Notes tab. Our page “Is my university NMC approved?” shows how to read a WDOMS entry, and each university’s page in our Russian university directory gives its FAIMER School ID where it has one.

The UK: GMC and PLAB

If your primary medical qualification is from outside the UK, the GMC must first accept it as an acceptable overseas qualification before you can take PLAB or apply for registration. Its criteria include these:

  • ›Criterion A: the awarding institution has a World Directory entry with contact details for verification, and the degree title, duration, awarding body and affiliations match that listing. The GMC “will consider the content of any sponsor notes”, but no GMC-specific sponsor note is required.
  • ›Criterion A also asks that the qualification lets the holder register to practise in the country that awarded it, including allowing a postgraduate internship there.
  • ›Criterion B: the programme runs over at least three years, and the GMC expects at least 5,500 clock hours of allopathic and clinical study.
  • ›Criterion E: most of the programme is taught in person.

Russian universities on the GMC’s lists

The GMC keeps a list of qualifications it may accept, assessed case by case, and asks holders to contact it before booking PLAB or applying. It names seven Russian institutions. Four of them are on our fee list.

From the GMC’s “may accept” list, checked 18 October 2026. The GMC uses its own names, and several universities have changed theirs, so we matched by institution and former names. We also have a page for Ryazan State Medical University, which is not on our fee list, and we have not matched it against the GMC’s Ryazan entry. If you study there, ask the GMC. Of its own list, the GMC says a place on it “does not mean that the qualification will not be acceptable”.

Crimea is a different case. The GMC’s list of qualifications it does not accept includes “Vernadsky Crimean Federal University S.I. Georgievsky Medical Academy (students who began their studies after 1 March 2014)”, filed under Ukraine with two older Crimean names under the same date condition. That is the Crimea Federal University (Medical Academy) on our fee list. Its WDOMS entry (F0000896) is also filed under Ukraine. It says the school “is located in the temporarily occupied territory of Ukraine” and that Ukraine does not recognise documents issued there, and it records that Russia’s Ministry of Education and Science recognises the degree. The entry shows an ECFMG note for graduation years 1953 to current, and a Canada note.

Before you book PLAB

The GMC tells candidates to check that it accepts their qualification before booking PLAB or applying. If you take PLAB and the GMC then does not accept the qualification, “your PLAB fees will not be refunded”, and “you can’t appeal to us against our decision”. If you apply for registration before the GMC has accepted your qualification, the GMC closes the application while it assesses the qualification, and the qualification has to pass primary source verification before registration. Before PLAB you also set up a GMC Online account and show that your qualification and your English are acceptable.

The GMC’s PLAB guide describes the two parts. PLAB 1 is 180 single-best-answer questions in three hours, and can be sat in a number of countries as well as the UK. PLAB 2 is an objective structured clinical examination (OSCE) of 16 scenarios of eight minutes each, run at the GMC’s clinical assessment centre. PLAB checks that you know and can do what a doctor starting the second year of the UK Foundation Programme can. After both, your application for registration with a licence to practise must be approved within two years of passing Part 2. Once registered you can work as a doctor in the UK; the guide does not promise a training post.

The UAE and Saudi Arabia

The UAE has three health regulators. The Dubai Health Authority’s (DHA) licensing manual, version 1.2 issued on 8 May 2025, names them: the Ministry of Health and Prevention (MOHAP), Abu Dhabi’s Department of Health (DOH) and the DHA. DOH’s own 2017 manual describes it as “previously known as the Health Authority - Abu Dhabi (HAAD)”. The DHA manual describes the Unified Healthcare Professional Qualification Requirements (PQR) as setting the qualification requirements for licensing health professionals in the UAE.

Paragraph 5.3 of the DHA manual asks you to meet the PQR on qualifications, experience, a licence from your home country, good standing, an assessment and primary source verification (PSV). In Dubai the steps run through the DHA’s Sheryan platform: an account and self-assessment, a positive PSV, the PQR assessment (a computer-based test, or an oral assessment after the registration application), then a Professional Registration Certificate. Those steps are Dubai’s; DOH and MOHAP run their own. We did not verify how many years of experience the PQR asks of a general practitioner, so take that figure from the regulator.

In Saudi Arabia, the Saudi Commission for Health Specialties (SCFHS) registers practitioners through its Mumaris Plus platform. It reviews “qualifications and experience to assess the ability to practice safely”, and the outcome is a direct decision, a practical assessment, an electronic written exam (through Prometric or Pearson VUE) or the national electronic licensing exam.

Practising in Russia

Article 69(1) of Federal Law No. 323-FZ, Russia’s law on the basics of health protection, gives the right to practise medicine in Russia to people educated at Russian institutions who have passed specialist accreditation. Under Article 69(3), an accreditation commission holds it when training ends, and it is repeated at least once every five years. We read the law in the ConsultantPlus consolidated text, edition of 4 August 2026, because the official portal did not open for us.

Primary accreditation follows the Regulation on Accreditation of Specialists, approved by the Health Ministry’s order No. 709n of 28 October 2022, in force from 1 January 2023 to 1 January 2029. It covers graduates of state-accredited specialist-degree programmes and people with specialist-level medical education from foreign institutions (para 2). The order’s text sets the rest:

  • ›It is held in Russian (para 69), on the premises of accreditation centres (para 68).
  • ›For clinical medicine there are three stages, taken in sequence: a test, practical skills in simulated conditions, and situational tasks (para 69). You move to the next stage only after passing the one before (para 71).
  • ›Each stage is passed at 70% or more, and on the practical stage that means 70% on every task (paras 74, 76 and 78).
  • ›You apply with your education documents or an extract from the State Examination Commission's protocol, so accreditation comes after the State Final Attestation. A foreign national's SNILS, Russia's individual insurance account number, is needed only "if available" (para 61).

Two questions are still open. A law of 17 November 2025 (No. 424-FZ) added parts 3.3 to 3.5 to Article 69: up to three years of mentoring after a first primary accreditation in specialties on a Health Ministry list, for people who finish training after 1 March 2026. We did not check whether General Medicine is on that list. And passing accreditation does not by itself settle whether an Indian national can be employed as a doctor in Russia. That depends on work-permit and immigration rules we have not verified.

What to get in writing before you join

Every route above runs back to the documents your university gives you. Ask for these:

  1. The split between course and internship, certified by the university: at least 54 months of study and a separate 12-month internship at the same university (FMGL-2021 regulation 4(a); FMGL FAQs, question 17).
  2. The medium of instruction for every year, the clinical rotations and the internship, and the university document that makes it English (FGOS clause 1.8).
  3. How a foreign graduate of the programme obtains the licence to practise in Russia that FMGL-2021 regulation 4(b) asks for.
  4. If the USA is a possibility, the ECFMG Sponsor Note on the university's WDOMS entry and the graduation years it covers.
  5. If the UK is a possibility, where the university sits on the GMC's lists: accepted, may accept, or not accepted.

Note

Before you pay, we give you in writing, for your university and programme: its WDOMS listing and FAIMER School ID, the study and internship split from the admission letter, the medium of instruction, and whether the degree licenses you to practise in Russia. What we do during the course is on our services page.

Note

A.J Trust Educational Consultancy compiled this page from the documents linked in each section and is not affiliated with NMC, NBEMS, ECFMG, Intealth, the GMC or any regulator named here. If something here does not match what you have been told, ask us.

Got Questions?

Questions people ask about careers after MBBS in Russia

Short answers. The sources are linked in the sections above.

Yes, if your degree meets NMC's Foreign Medical Graduate Licentiate Regulations, 2021 (FMGL-2021) and you complete the steps after it. FMGL-2021 asks for at least 54 months of study, a separate 12-month internship at the same Russian university, English as the medium of instruction, and registration to practise in Russia on the same terms as a Russian citizen. You then pass FMGE, do a 12-month internship in India and apply to your State Medical Council for permanent registration. If you were already studying abroad before 18 November 2021, the older Screening Test Regulations, 2002 apply to you instead.

Only if the university's own documents show it. Russia's federal standard for General Medicine (FGOS 31.05.01) sets a six-year programme with clinical practice built into it and names no separate internship year. NMC's FMGL FAQs (question 17) say the internship is a separate phase, done after the 54-month course. Before you join, ask the university for a signed statement of how many months are study and how many are internship.

Yes. Clause 4.10 of the NEET-PG 2026 bulletin asks a candidate with a foreign medical degree for an FMGE pass, registration with NMC or a State Medical Council, and an internship completed or due to finish by the session's cut-off, which was 30 September 2026 for NEET-PG 2026. So you can apply while the internship in India is still running, as long as it ends by that date. In a tie, your FMGE percentage is used where an Indian graduate's MBBS aggregate would be, after six other tie-breakers.

It depends on the university, not on Russia. USMLE Step 1 and Step 2 CK are open to students and graduates of a school that WDOMS lists as meeting ECFMG's eligibility requirements, and ECFMG Certification needs an ECFMG Sponsor Note on that school's WDOMS entry covering your graduation year. Certification also needs Step 1, Step 2 CK and an OET Medicine score through an ECFMG Pathway. Step 3 needs ECFMG Certification first.

Only after the GMC accepts your degree. The GMC assesses seven Russian institutions case by case and asks their graduates to contact it before booking PLAB: Far Eastern State Medical University (transfer students only), Mari State University Medical Faculty, North-Western Medical University named after I.I. Mechnikov, Privolzhsky Research Medical University, Ryazan State Ivan Petrovich Pavlov Medical University Medical Faculty, St Petersburg Pavlov State Medical University and St Petersburg State Pediatric Medical Academy. It does not accept the S.I. Georgievsky Medical Academy of Vernadsky Crimean Federal University for students who began after 1 March 2014. If you sit PLAB and the GMC then does not accept your degree, your PLAB fees are not refunded.

No. NMC deferred NExT on 13 July 2023 "till further directions" from the Health Ministry, and on 18 October 2026 its What's New list had no NExT notice later than 8 February 2024. No start date or first batch has been notified. NMC's draft amendment of 11 August 2026 would name FMGE as the route for foreign graduates until NExT comes into force, but on 18 October 2026 it was still a draft.

Two years from passing FMGE, under Schedule I of the Compulsory Rotating Medical Internship (CRMI) Regulations, 2021. There are outer limits too: NMC's public notice of 7 December 2023 says all foreign medical graduates must complete their internship within 10 years from the date of joining their foreign medical institution. NMC's FAQ of 8 June 2026 adds that a graduate admitted abroad after 2019 must clear the FMG examination within 10 years of admission. Count both from the day you joined in Russia.
